Important Health Requirements for the 2025/26 School Year

Dear GISNY Families,

As we prepare for the upcoming school year, we want to remind you that the health and safety of our community remain our top priority. Please take a moment to review the important health requirements outlined below to help us maintain a safe and healthy environment for all. School Health Examinations

We strongly encourage you to schedule appointments with your child’s pediatrician early, as medical offices tend to be especially busy during late summer and fall.

Your pediatric office will provide you with the School Health Examinations Forms, which must be completed and uploaded to Magnus Health.

 

Health Examination Requirements – As Mandated by New York State Law

A current health examination form is required for:

  • All new students entering GISNY
  • Students in Pre-K, Kindergarten (ES), Grades 1,3,5,7,9,11 (i.e., every other year)
  • Annually for all students participating in interscholastic athletics (beyond PE)
  • Annually for all students applying for a worker permit (Youth Working Permits)
  • When requested by the Committee on Special Education 

 

Immunizations

New York State law requires all students entering or attending any NYS school to receive all doses of immunizations required for their grade level to attend school. The immunization requirements are outlined on the NYSDOH Immunization Requirements for School Entrance and Attendance Chart.

Important immunizations are coming up before entering grades K ( = ES), 6,7 & 12.

Kindergarten / Eingangsstufe:

  • MMR (Measles, Mumps, Rubella) booster
  • Varicella (Chickenpox) booster
  • IPV (Polio) booster – must be administered after the 4th birthday
  • DTaP (Diphtheria, Tetanus, Pertussis) booster – must be administered after the 4th birthday

6th Grade:

  • Tdap (Tetanus, Diphtheria, Pertussis) booster – must be administered after the 10th birthday

7th Grade:

  • Meningococcal ACWY vaccine – must be administered after the 10th birthday

12th Grade:

  • Meningococcal ACWY booster – must be administered after the 16th birthday

 

Please upload all updated immunization records to Magnus Health by August 15, 2025.


 

Prescribed AND Over-the-Counter Medications

If your student needs to take medication during the school day, the following requirements must be met before the school can administer or permit self-administration of any medication:

Medication Permission Form must include:

  • An order from a licensed healthcare provider
  • A signed statement from the parent or guardian granting consent
  • Please note: A provider’s written order is required even if the student only uses OTC medication and will self-administer.

Medication Delivery:

  • Medication must be in its original, properly labeled prescription or over-the-counter container.
  • Any special equipment needed to administer the medication must also be provided.
